My girlfriend does triathlons and qualified for Kona last year. I made her a custom jersey last year; one of her friends saw it, loved it, and my girlfriend promised her that if she qualified for Kona I'd make her a jersey.
The hardest part of doing this jersey was pinning down what she wanted. There was no specific design in mind, I was given some elements she wanted - Kanji Dream characters, Sunflowers, Carpe Diem, and because he's da bomb Curious George. I didn't get the Carpe Diem in, but I did get the rest. The client was a bit concerned about it being a white jersey (wet white Tee, uh oh), so I wound up painting the whole front. No word on the final image, but she was very impressed with the WIPs.
In the end, I finally decided to iron the shirt to do the heatsetting. Its not supposed to be ironed, so I made sure to move the iron around a lot. That got the shirt pretty hot, but I don't know if it was really enough to do a proper setting according to the E'Tac guidelines. I also used a coat of Save-Tee over that. If its washed correctly, hopefully it will hold up for a long time.
